چکیده

Sjögren’s syndrome (SS) is a systemic autoimmune disease that mainly affects the exocrine glands and usually presents as persistent dryness of the mouth and eyes due to functional impairment of the salivary and lacrimal glands [1, 2]. The common histolopathological feature of all organs affected is a potentially progressive lymphocytic infiltration. Salivary glands are the most studied organs because they are affected in almost all patients and are easily accessible. Microscopic examination of the salivary glands reveals a benign lymphoepithelial lesion, characterized by lymphocytic replacement of the salivary epithelium and the presence of epimyoepithelial islands composed of keratin-containing epithelial cells. The predominant cells in the minor labial salivary gland infiltrates are T cells, with a bias towards CD4þ cells rather than CD8þ suppressor cells (CD4/CD8 ratio of 3:1–5:1). B cells constitute approximately 20% of the total infiltrating population, while natural killer (NK) cells are observed less often (5%) [3]. The aetiopathogenesis of primary SS is probably a sequential, multistep process that leads to selective damage of the exocrine glands, with consequent target organ dysfunction. Although the exact mechanisms involved in this aetiopathogenic process are not well known, the autoimmune origin of the disease (autoimmune epithelitis [4]) is probably the aetiopathogenic hypothesis most commonly postulated in primary SS. In this review we have summarized the current concepts on autoimmune aetiopathogenesis of primary SS and reviewed alternative aetiopathogenic mechanisms that have recently been postulated.

Current Concepts: Mouse Models of Sjögren's Syndrome

Sjögren's syndrome (SjS) is a complex chronic autoimmune disease of unknown etiology which primarily targets the exocrine glands, resulting in eventual loss of secretory function. The disease can present as either primary SjS or secondary SjS, the latter of which occurs concomitantly with another autoimmune disease such as rheumatoid arthritis, systemic lupus erythematosus, scleroderma, or prim...

Primary Sjögren's syndrome: new clinical and therapeutic concepts.

Sicca features are the central clinical manifestations of Sjögren's syndrome (SS), but recent studies have confirmed that primary SS has a systemic expression, including extraglandular manifestations. Patients with a predominantly extraepithelial expression should be managed differently from patients with predominantly periepithelial or sicca limited disease. In coming years treatment will be b...

Eligibility for clinical trials in primary Sjögren's syndrome: lessons from the UK Primary Sjögren's Syndrome Registry.

OBJECTIVE To identify numbers of participants in the UK Primary Sjögren's Syndrome Registry (UKPSSR) who would fulfil eligibility criteria for previous/current or potential clinical trials in primary SS (pSS) in order to optimize recruitment. METHODS We did a retrospective analysis of UKPSSR cohort data of 688 participants who had pSS with evaluable data. RESULTS In relation to previous/cur...

How to assess treatment efficacy in Sjögren's syndrome?

PURPOSE OF REVIEW This article critically reviews the current views and discusses the future challenges with regard to assessing disease progression and disease activity in Sjögren's syndrome, as a decrease of disease progression and activity is what an effective Sjögren's syndrome therapy aims for. This topic has recently gained renewed attention as targeted treatment modalities have become av...

Importace of monoclonal immunoglobulin in Sjögren’s syndrome

Background: Sjögren’s syndrome is the second most common systemic autoimmune disease after rheumatoid arthritis, RA, with a prevalence of about 0.5% in the general population. It occurs primarily in perimenopausal women (at a ratio of women to men of 9: 1) Sjögren’s syndrome is characterized by lymphocytic infiltration to exocrine glands. The specific autoantibodies of this syndrome are against...
